Please arrive 5-10 mins early to complete intake forms. You can also print the attached and bring with you if you prefer. 

Please wear or bring shorts/underwear you are comfortable being seen in, as there will likely be movement and position changes during the session.

I am able to bill most MVA/PIP insurance claims. Unfortunately I am not able to bill health insurance. By request, I can provide a “superbill” for you to submit to your insurance for possible reimbursement. 

Cash, check, Venmo accepted for payment. I also accept HSA flex spending cards. 

Turn Right on Spruce when you arrive from maps. The clinic is in the lower unit of a duplex. You can park in the driveway near the blue Subaru. 

The clinic is in a daylight basement ADU in a residential neighborhood. Plenty of easy street parking.

Tipping is not expected. Any payment above the shown rates goes to support my sliding scale work.
